Scholarship Application

Selection Criteria

To be considered, scholarship applicants must:

§ Be a woman pursuing an associate, bachelor or graduate degree.

§ Be a United States citizen or have been granted permanent residency.

§ Have completed at least 30 semester hours of credit as of 12/31/2008 in a college or university in the State of Texas.

§ Have a minimum cumulative grade point average (GPA) of 3.0 on a 4.0 scale.

§ Be enrolled to take six or more credit hours in a college or university in the State of Texas.

§ Submit a complete, signed application, together with all supporting documentation, by the deadline date listed under the Deadline Date section.

§ Submit this mandatory supporting documentation

1. Proof of U S residency

2. Documentation from educational institution verifying tuition/fees

3. Statement explaining reason applicant is applying for scholarship, in 200 words or less

4. Statement of short and long term goals, in 200 words or less

5. Most recent transcript (issue to student is acceptable)

6. Two letters of recommendation

7. 200 word or less biographical sketch (appropriate for inclusion in Women of Excellence Awards Gala souvenir program)

8. Digital photograph in jpg format

Selection Process

· The scholarship recipients will be selected by the Foundation Governing Board.

· Members of the Foundation Governing Board and their dependents are not eligible to receive scholarships.

· It is the applicant’s responsibility to submit a complete and signed application and all the supporting documentation in one package which must be received on or before the deadline date as set forth below.

· The Foundation Governing Board accepts no responsibility for incomplete applications, applications not in proper form, lost applications, or any other submission in exception to the above instructions.

· The scholarship award will be based on the committee’s scores of the overall quality of the application and the supporting documentation.

Deadline Date

· Completed application and supporting documentation must be postmarked or emailed on or before May 15, 2009. Send either by mail or email, but not both.

· Applications received after the designated deadline will not be considered. No exceptions.

Award Notification

The Foundation Governing Board will send email notification by June 15, 2009 to scholarship recipients.
